invitation blank page

I've followed the demo site configuration but I've a blank page when clik on invitation
referral, credit history works fine but not invitation
set to published and form type email

Not sure what the problem might be. Can you go to global configuration and temporarily set:
- debug mode to yes
- error reporting to maximum

and then refresh the page? What error do you see?
