Gift Certificate coupons are not generating

Upon the purchase of a gift certificate product, no coupons or codes are generated and an email is not sent to the user's account with the gift certificate.

The coupon template and gift certificate product is set up as expected and I've double checked against the the tutorial provided.

What would be the reasons for this?

I solved this by creating my own profile entry; strangely, none of the default profiles were showing in the profile menu, but they were in the drop down for when creating a gift certificate. Assigning a custom profile to the gift certificate enabled the coupon generation and email.
